* third parties mandated to perform certain tasks in connection with the analysis of your data to enable better diagnosis and treatment (laboratory technicians, firms offering data analysis services using physiological data recording software, forms using artificial intelligence to analyze certain medical data, etc.);
* referring, treating or interpreting physicians, including the company's medical director;
* companies offering you benefits or compensation for our products and services(insurers, Veterans Affairs Canada, CNESST, SAAQ, NIHB, local employment centers,etc.);
* third parties who provide IT, Web and data hosting services;
* third parties who offer home delivery services, who may have access to the personal information we hold, strictly for the purposes of the services they provide to us;
* government authorities and regulatory bodies, including the Ordre des inhalothérapeutes du Québec, the Collège des médecins du Québec and the Ordre des Infirmières Et Infirmiers Auxiliaires du Québec, when required by law;
* professionals providing services to our firm (legal advisors, financial advisors,insurance brokers and agents, bankers, accountants).
I also consent to the transfer of my personal information outside Quebec for the specificpurposes mentioned in this consent.
Destruction of my personal information
I authorize Innovation CPAP to destroy my personal information in accordance with the law and the terms of the Privacy Policy adopted and published by the company.
Rights regarding my personal information
I declare that I have been informed of my rights with respect to my personal information, including my right to access, rectify and challenge the use of my personal information. I also understand that I may withdraw my consent at any time, in whole or in part, by giving written notice to Innovation CPAP.
Period of validity of this consent
This consent is valid for as long as is necessary to achieve the purposes for which the consentis given, i.e. the provision of the professional services I require and/or the purchase of Innovation CPAP products.
I acknowledge that this consent is given in accordance with the Act respecting the protection ofpersonal information in the private sector and the Privacy Policy adopted and published by Innovation CPAP.
